inFORME healthcare has been a leading provider in the post-acute care continuum since 2014. We were founded to meet the growing demands in the transition from acute care hospitals to post acute care. Our clinical services platform is designed for an Innovative Focus on Optimal Results through Medical Excellence (inFORME).

Internal Medicine

The foundation of our programs starts with stellar primary care. We provide a highly qualified medical team consisting of Physicians and Advanced Practice Providers integrated in each location.

Telemedicine

When not on-site, we continue to provide care via telemedicine. Our program was intentionally designed to augment quality and mitigate risk to patients who may need services when we are not in house.

Wound Care

We create value by integrating team members providing cohesive patient care. Our interdisciplinary team includes highly skilled wound providers who evaluate and manage wounds to expedite recovery.

Palliative Care

Our interdisciplinary team optimizes medical care and quality of life to manage complex conditions that can cause pain.

Transition of Care

A unique post-acute continuum to bridge and optimizing care between locations.  Patients move fluidly through the care continuum with oversight by our medical team for timely and effective care.

Behavioral Health

We believe in psychosocial well-being. Our team includes providers that are trained in behavioral health and counseling to address health and well- being of mind and body.

Clinical Education

We partner closely with our customers to elevate clinical standards. Our team will provide clinical education in the form of quarterly in-services.

Pulmonology

Our pulmonology specialists focus on respiratory health. They aim to diagnose, treat, and prevent medical conditions that affect the lungs and respiratory tract.

COVID-19 Management

We are adept at managing Covid-19 and have successfully handled caseload surges of over 800 positive patients per day across multiple states. Our medical executive committee continually collaborates with leading infectious disease experts and virologists to update our protocols on the latest treatment standards.

Internal Medicine/ Hospitalists

Medical specialists dealing with the prevention, diagnosis, and treatment of internal diseases.

Infectious Disease

Medical specialists who treat acute and chronic infections caused by bacteria, parasites, fungi and viruses, including COVID-19.

Pulmonologists

Special training in diagnosing and treating diseases of the lungs.

Palliative Care

Trained to prevent and ease suffering for people who have serious illnesses or who need end-of-life care.

Physiatry

Also known as Physical Medicine and Rehabilitation (PM&R) treat a wide variety of medical conditions affecting the brain, spinal cord, nerves, bones, joints, ligaments, muscles, and tendons.

Psychiatrists and Psychologists

Our behavioral health team specializing in the diagnosis and treatment of mental illness including comprehensive counseling.

Nurse Practitioners

An advanced practice registered nurse trained to assess patient
needs, order and interpret diagnostic and laboratory tests, diagnose disease, formulate and prescribe treatment plans.

Physician Assistants

Advanced practice provider who is trained to diagnose illnesses, develop and manage treatment plans, prescribe medications, and may serve as a principal healthcare provider.

Behavioral Health Counselors

Licensed professionals who help people manage and overcome mental and emotional disorders.
